This includes if your child is promoted. Once you withdraw your preferred time and day may not be available should you decide to re-join at a later date. You can withdraw from a class by submitting this form no later than two (2) days before the next scheduled debit date. Debit dates will be the third Monday of each month for payment in advance for the next calendar month's lessons. Accounts that are cancelled will forfeit all unused makeup lessons from the date of withdrawal. If this form is submitted after, or less than two (2) days before, scheduled debit dates please note that no credits, makeups or refunds will be issued for missed or unused classes.
